Rt Hon MP, David Lammy, visited the Haringey Multi-Agency Care and Coordination Team (MACCT) at Lordship Lane Primary Care Centre last week. 

The MACCT brings together over 25 professionals from across the NHS, social care and the voluntary sector, including our Care Navigators, to deliver proactive support for adults living with frailty or complex conditions.  

In the past year alone, the team has supported 2,516 people, reducing hospital demand by 30% and delivering over £1 million in system savings, while also improving outcomes and reducing health inequalities.
