This September, we are celebrating 10 years of our Home from Hospital Service delivered in partnership with Haringey Council and the ICB. We've received referrals from across the borough, working closely with nurses and doctors from North Middlesex, Whittington, and UCLH, as well as GP surgeries, to ensure their patients can return home as soon as possible and receive the support they need to stay there. 

Over the past decade, the service has supported more than 500 people every year to feel less alone and regain independence after a hospital stay. From practical help with shopping and medication to emotional reassurance and signposting to community services, our dedicated staff and volunteers have made a real difference to the lives of many Haringey residents.

Comfort and Care: Mr. RS’s Story

When Mr. RS was discharged from Whittington Hospital, the Home from Hospital (HFH) team escorted him home and helped him settle in, providing practical and emotional support. A week later, on a follow-up visit, the HFH team found Mr. RS lying on the floor after a fall. Over the following weeks, the HFH team worked closely with his GP and hospital staff to ensure he received the care he needed. With a carer in place and new safety adaptations at home, Mr. RS began to feel more supported and less isolated.

“Until your service found me, I felt invisible. Thanks to you, I no longer feel lonely. My house is clean, I have a carer, and I believe your visit even saved my life.”  

By helping to discharge patients safely as soon as possible, our service is also helping relieve the pressure on the NHS by reducing unnecessary bed blocking when patients are waiting for support, as well as reducing the risk of patients being readmitted.
